Life Care Planner

Innovative non-profit care management organization is expanding its Medical Legal Services Division seeking certified Nurse Life Care Planners to join our team. This new position offers nursing professionals (BSN/MSN with CCM) an opportunity to use their expertise in the coordination of care for individuals while learning to develop comprehensive life care plans for individuals with complex medical needs. Once certified in life care planning, your career continues to expand to include providing expert testimony nationally.

OUR POSITION:
• Utilizes the nursing process in the collection and analysis of comprehensive client specific data to prepare life care plans developed for individuals who have experi-enced an injury or have chronic healthcare issues

• Provides expert testimony at deposition and in court when required

• Develops policies and procedures relevant to scope of practice

• Identifies new options, expansion of service area and enhancement of current
services

ESSENTIAL QUALIFICATIONS:
• Extensive experience in rehabilitation for individuals with catastrophic illness/injury
• Prior experience in in life care planning or care management
• RN with BSN or other health care baccalaureate degree
• Certification in life care planning or certified case manager
