The Center provides care coordination services in the community among

• Persons with complex medical needs
• Those who are dependent on life sustaining technology
• People with catastrophic illness
• Individuals with special health care needs and disabilities

Driven by our mission, The Coordinating Center is committed to supporting individuals of all ages to live as they wish, considering health and safety, in their own homes and communities. In Maryland, The Center provides comprehensive care coordination through

  • The Maryland Medicaid Model Waiver for children with complex medical needs and disabilities
  • The Maryland Rare and Expensive Case Management program (REM) for people of all ages with the most challenging health concerns

The Center also provides innovative service coordination among

  • Young children participating in the Infants and Toddlers program in several Maryland jurisdictions
  • Children benefiting from the Autism Waiver Program
  • Adults with physical disabilities aged 60 and under, who are transitioning from nursing facilities to homes of their own in the community through the Living at Home Waiver program.

Additionally, The Center serves individuals identified by Amerigroup, a Medicaid certified managed care organization, to provide care coordination for people who have extraordinary health needs. 

What is different about The Coordinating Center? The staff reflects broad expertise in nursing, social work, community resource development, special education, and rehabilitation. Essential to the family and person centered approach is expertise in cultural competency and sensitivity to cultural perspectives in health and disabilities.  The organization has pioneered a care coordination system that relies on a dynamic relationship between families and individuals, providers and professionals.  The Center offers a holistic approach by coordinating all of the services a person needs to be fully included at home and in the community. Because coordinators use appropriate community resources to help people build networks of support, the staff is able to promote more effective, more cost efficient strategies to reach positive outcomes.

The Coordinating Center’s care coordination program has received accreditation by URAC/American Accreditation HealthCare Commission under its Case Management program. The Center is among a small number of non profit organizations to be recognized by this national accreditation agency. The care coordination provided to pediatric clients has been recognized by the American Academy of Pediatrics in its Guidelines for Pediatric Home Health Care.
