Care Coordination, REM Program The Rare and Expensive Care Management Program (REM) is a Maryland Medicaid program that is part of Maryland's managed care system known as HealthChoice. This program is intended to provide specialized services to people with extraordinary health care needs. People who may benefit from REM are those with a diagnosis or care needs that are beyond the usual scope of Medicaid managed care companies. To participate, aside from Medicaid HealthChoice eligibility, the person must have a diagnosis that is included in the program's diagnosis list. This list is developed by the Maryland Department of Health and Mental Hygiene (DHMH) which administers the program. Information regarding these diseases or conditions may be obtained by calling 1-800-565-8190. One of the services that everyone participating in REM receives is care management. The Coordinating Center is one of the organizations providing this service. As individuals apply to the program, DHMH assigns case management organizations to work with that person. However, both physicians and families or individuals may request a care management organization of their choosing. What is different about Coordinating Center care management? The Center believes in working with individuals and families in a partnership. We build community support networks to assist people to be fully included at home and in the community while working to promote optimal health and safety.
