The Coordinating Center is excited to announce that Shenita Freeman is the Program Director for the Wellness and Independence for Seniors at Home (WISH) program.
Ms. Freeman brings more than 11 years of clinical and administrative healthcare experience to The Coordinating Center. As the Program Director of WISH, she will oversee the team of health coaches, a RN Care Coordinator, a Community Program Liaison and Coordination Specialist. As part of the Nexus Montgomery Regional Partnership, WISH aims to reduce avoidable hospital use by people age 65+ living independently in Montgomery and parts of Prince George’s counties who are in jeopardy of declining health.
Ms. Freeman holds a Bachelor of Arts in Chemistry from Metropolitan State University, a Master of Public Health in Epidemiology from Colorado School of Public Health, and is pursuing a Master of Computer Information Systems – Health Informatics from Boston University.

The Coordinating Center is excited to announce its 2nd Annual CenterFlix will be on November 7, 2016 at the Baltimore Museum of Art. The event will include feature films from Sprout Film Festival, live performances, visual art, a raffle and cocktail reception.
All CenterFlix proceeds will benefit The Center’s Family Resource Fund, which provides critical financial assistance for children and adults with disabilities and chronic conditions.

The Coordinating Center is excited to announce that Lynn Baklor is the Senior Vice President, Finance & Administration and Chief Financial Officer.
Ms. Baklor brings more than 25 years of experience in executive level management, strategic planning and financial analysis to The Coordinating Center. As the Senior Vice President, Finance & Administration, Ms. Baklor will be part of the executive team and is responsible for overseeing the Accounting and Offices & Facilities divisions.
Prior to joining The Coordinating Center, Ms. Baklor provided corporate finance and operations consulting services for companies and entrepreneurs. She was previously the Director of Finance for Under Armour, Inc.
Ms. Baklor holds a Bachelor of Arts degree from Duke University and a Master of Business Administration from the Wharton School of Business, University of Pennsylvania. She is the Chair of the Leadership Alliance and Board President of The Red Devils – Supporting Breast Cancer Families.

The Coordinating Center is expanding its evidenced-based care coordination services to be part of the movement to transform population health in Maryland. The Coordinating Center is excited to announce that it is the first state-wide, nonprofit care coordination agency to be part of two regional partnerships that will receive grant funding from the Maryland Health Services Cost Review Commission (HSCRC) for System Transformation. A total of nine awards equaling just over $30.5 million were awarded to help Maryland hospitals meet the goals of the new All-Payer Model and Triple Aim.
Working with local hospitals and other community organizations, The Coordinating Center is a proud partner of the Nexus Montgomery Regional Partnership and the Bay Area Transformation Partnership.
As part of the Nexus Montgomery Regional Partnership, The Coordinating Center will provide health coaching and care coordination through the Wellness and Independence for Seniors at Home (WISH) program. Working with Anne Arundel Medical Center and the University of Maryland Baltimore Washington Medical Center, The Coordinating Center will provide health coaching and care coordination to individuals who are at risk for hospitals encounters and have Medicare or who are dually eligible for Medicare and Medicaid as part of the Bay Area Transformation Partnership.
With more than 33 years of experience in delivering community-based care coordination with people with disabilities and the most complex medical and social needs, The Coordinating Center has the expertise needed to help the state of Maryland reduce per capita hospital expenditures and improve health outcomes. “We are looking forward to partnering with the hospitals and community organizations in the Nexus Montgomery Regional Partnership and the Bay Area Transformation Project to help individuals throughout the state meet their health goals,” said Carol Marsiglia, Senior Vice President, Strategic Initiatives and Partnerships. “Our results show that community-based care coordination identifies health and social risks that can be addressed to avoid hospital encounters, resulting in better health outcomes, increased customer satisfaction and lower costs to our health system.”

The Coordinating Center has been granted full Case Management Accreditation Version 5.0 from URAC, a Washington, DC-based health care accrediting organization that establishes quality standards for the health care industry. The accreditation is valid for three years.
The Coordinating Center is pleased to announce the appointment of Karen-Ann Lichtenstein, President/CEO, as the Chair of the Maryland Commission on Disabilities by Governor Larry Hogan. The Maryland Commission on Disabilities (MCOD) serves as the advisory body for the Maryland Department of Disabilities, which is charged with unifying and improving the delivery of services to people with disabilities in Maryland.

The West Baltimore Health Enterprise Zone (HEZ)/West Baltimore Care has contracted with The Coordinating Center to reduce disparities and improve health outcomes among underserved communities and reduce health costs and avoidable hospital encounters through its care coordination program.
The Coordinating Center provides innovative delivery of care transition work and has a proven track record of reducing readmission rates by more than 20 percent in three West Baltimore hospitals and producing an average of 149% return on investment (ROI) for the hospitals. (more…)
