The Coordinating Center is participating in the Chronic Obstructive Pulmonary Disease (COPD) Readmission Summit: Integrating COPD into Patient Centered Hospital Readmission Reduction Programs on October 11, 2013 in Washington, D.C.

According to the Centers for Disease Control and Prevention, COPD is the third leading casue of death in the United States.

The Coordinating Center will be part of the “Fitting COPD into National Hospital Readmission Reduction Models” session. The Center will discuss its participation in the the Community Care Transition Program (CCTP)  – West Baltimore Readmission Reduction Collaborative (WBRRC), the use of the Coleman Care Transitions Intervention Model and how COPD fits into the model.

The goal of the CCTP is to have 20 percent reduction in 30 day all cause hospital readmissions. An average of 20 percent of patients hospitalized with COPD exacerbations nationally are readmitted within 30 days, reports the Healthcare Cost and Utilization project.

According to The Coordinating Center’s President and CEO, Karen-Ann Lichtenstein, “With such a high percent of the individuals discharged from the WBRRC hospitals having COPD or other chronic lower respiratory diseases, our Transition Coaches are imminently poised to support individuals to remain healthy in their communities.”
