We specialize in addressing Social Determinants of Health (SDoH) by connecting individuals to vital resources such as housing, transportation, food security, employment, and social support. Our strategic interventions improve health equity and overall client well-being.Â
Managed Care Case Management
We partner with Managed Care Organizations to deliver case management for members with high-risk medical and psychosocial profiles. Our services reduce preventable hospitalizations, lower healthcare costs, and enhance member satisfaction by ensuring comprehensive, coordinated care.Â
Permanent Supportive Housing Coordination
Utilizing a Housing First model, we help individuals with disabilities, chronic health issues, and mental health conditions secure and maintain permanent supportive housing. Our coordinated approach links housing with wraparound services, promoting housing stability and improved health outcomes.Â
Medical Legal Services
