Since 1983, The Coordinating Center has been helping people living with disabilities and complex medical needs live healthier, more independent lives. Our Coordinators support individuals and their families by providing care, resources, and connections to help them reach their goals. We believe in putting people first and treating everyone with respect, dignity, and kindness.
How We Help
At The Coordinating Center, we work with people of all ages and abilities. Our team helps individuals and families:
- Share their needs and goals – We listen carefully to understand what matters most.
- Find the right support – We connect people to helpful programs and services in their community.
- Build strong support networks – We make sure individuals have the right people and resources to help them succeed.
- Navigate systems and services – We assist with eligibility, applications, and access to care.
- Respect personal choices and cultures – We honor each person’s background, language, and expectations by embracing CARE – Culture, Access, Respect and Equity.
Our goal is simple: to help people live the life they want with the support they need.
