Our Care Managers who are licensed nurses and social workers, help people living with disabilities and complex medical needs access affordable, medically appropriate services that fit their unique needs. By connecting people to the services they need—like medical care, community programs, and long-term support—we help them build stable, independent lives with the best possible quality of life. Our Care Managers guide individuals and their families through key life transitions, such as early childhood interventions, school changes, and shifts from pediatric to adult care.
