Rare and Expensive Case Management (REM) Program

The Rare and Expensive Case Management (REM) Program provides medical case management and other services for eligible individuals living with specific medical conditions that are rare in occurrence and/or expensive to treat. This program is for eligible individuals receiving Maryland Medicaid who voluntarily choose the REM Program as an alternative to being in a Managed Care Organization (MCO) in the community.

Once enrolled, participants are assigned a Clinical Care Coordinator to assist with creating a Care Management Plan to access the medical care and resources they need to meet their health goals. This Care Management Plan is developed in collaboration with the participant and their support team, which may include their family and/or caregiver(s), as well as their medical team. Participants benefit from ongoing support from their Clinical Care Coordinator, including monthly contacts and scheduled face-to-face visits in their home and/or community.

Covered Services

All REM Participants have access to case management services provided by a Clinical Care Coordinator who is a licensed nurse or social worker dedicated to their health and wellbeing. In addition, the REM Program provides a wide variety of Medicaid funded services, such as physician visits, therapies, dental care, and pharmacy. Optional services such as private duty nursing and nutritional supplements may be funded if the participant has a medical need and meets other requirements. Medically necessary services must be ordered by a participant’s healthcare provider (with an active Maryland Medicaid number) and may require preauthorization. It is the role of the Clinical Care Coordinator to guide participants through accessing covered services.

Population Served and Eligibility

  • Must be eligible for community Medicaid
  • Must have a REM qualifying diagnosis (See COMAR 10.09.69.17)
  • Age criteria linked to qualifying diagnosis:
    • The REM program serves ages birth through 65, however each qualifying diagnosis has its own covered age limit.
  • Individuals may not be eligible for the REM program if they have Medicare.
