Our Community Health Team works with local health and human service agencies, hospitals, community providers and health plans to make sure people receive the care they need at the right time and place. Through Community Health programs, The Coordinating Center helps people move smoothly between different healthcare settings, like from the hospital to home, and make sure they do not miss important care. Expert coordinators help with everyday needs that affect health, like finding affordable housing, healthy food, and job opportunities. The Coordinating Center’s goal is to help people live healthier, by connecting them to the right services and resources.
