The Coordinating Center’s Homeless Support Services Division helps individuals facing housing insecurity gain access to supportive services such as behavioral health, home and community-based services, aging and disability services, healthcare, vocational services, and transitional services, including shelter.
Our aim is to ensure individuals receive the care they need to become independent and move towards self-sufficiency. Ongoing efforts include partnerships with local government and community-based organizations to improve access to services, identifying gaps, and enhancing coordination to better serve individuals with complex needs.
Our Service Coordinators have extensive experience working with individuals of all ages and abilities, including those who have complex medical and behavioral health needs. Our team supports individuals in achieving their aspirations for independence, recovery, and a meaningful life by addressing gaps in care, skills, and social determinants of health.
How We Help:

Provide intensive community-based care coordination to assist individuals in accessing appropriate medical, behavioral health, and social services, including access to recovery and treatment services.

Encourage self-sufficiency, through skill development and access to transitional services, including vocational training.

Help individuals learn how to be fully included in their community again through coaching and training.

Provide resource coordination for assistance around basic needs, such as food, household furnishings, and transportation.

Link individuals to community support and resources, they choose to provide them with opportunities for continued growth and success.
