Service Coordination

Our Service Coordinators who are Supports Planners and Coordinators of Community Service help people living with disabilities and older adults access home and community-based services. Our Service Coordinators use a person-centered approach to help people of all ages and abilities navigate complex medical, social, and educational systems, connecting them with the services they need to live independently and thrive in the community of their choice.
