The Coordinating Center partners with the Maryland Developmental Disabilities Administration (DDA) to provide Coordination of Community Services (CCS) for individuals of all ages with intellectual and developmental disabilities and their families. Our expert Coordinators are here to help you plan for the future and achieve your personal goals.
Through the CCS program, our Coordinators help you create a person-centered plan based on your goals for a meaningful life. We support you in making informed choices about your care and connect you with service providers and community resources. Our goal is to ensure you have the support you need to move forward with confidence.
How can The Center help you?
- Eligibility Assistance: Guide you and your family through the DDA eligibility determination process once DDA has determined eligibility.
- Waiver Enrollment: Help you secure and retain enrollment in DDA’s Community Pathways Waiver.
- Transitioning Youth Services: Facilitate your transition to adult services and other types of DDA services.
- Self-Direction: Help navigate the process of self-directing services.
- Waiting List Coordination: Connect you with community resources while waiting to be enrolled into the DDA waiver program.
