Supports Planning Services

Supports Planning helps connect all the pieces of your care.

There are a lot of steps to getting services—from applying to actually receiving them—and a Supports Planner is there to guide you through the whole process. They are your advocate and partner, making sure your program and services works for you, and are there to support your goals for independence.

Supports Planners help individuals apply for and receive both Medicaid and non-Medicaid funded home and community-based services through Long Term Services and Supports (LTSS). They support older adults and people living with disabilities or chronic illnesses to make choices about where and with whom they live, have control over the services they receive and who provides them, pursue work, and involve friends and support networks to stay engaged in their communities.

Population Served and Eligibility

As a Supports Planning Services Provider for Maryland Medicaid, The Coordinating Center provides support to eligible participants, Medicaid recipients who require assistance with activities of daily living who have a chronic illness, medical condition or disability. The program supports eligible participants in either staying in the community they choose or transitioning from a Maryland nursing facility to a home setting within their preferred community.
