Providing effective care coordination can be challenging and complex, especially for people with special health care needs, including but not limited to chronic diseases, disabilities, cognitive delays, behavioral health who receive care from numerous providers and systems. That is why VIP Connect partners with primary and specialty care providers, the patient, and their family to deliver a collaborative self-management, care coordination model. As a participant, you will have access to our experienced care team to help you address barriers in care, develop a focused needs plan, and find resources that best meet your needs.
